Outreach Service

Inclusive Care Support offers community-based outreach support and home-based care, helping individuals maintain independence, access meaningful activities, and build confidence in everyday life.

What we do

At Outreach Care Support Limited, we believe that everyone deserves the opportunity to live as independently and joyfully as possible—whether in their own home or out and about in their community. Our support is carefully tailored to each individual’s needs, abilities, and aspirations.
Our services include:
Community engagement support: helping individuals access local groups, social events, educational opportunities, and leisure activities.
Home-based care: supporting with daily living tasks such as meal preparation, personal care, medication prompts, household routines, and companionship.
Independence building: developing practical life skills, travel training, budgeting, and decision-making support.
Flexible, person-led planning: working with individuals, families, and professionals to ensure the right level of care and involvement.
We support children, young people, adults, and older people—including those with learning disabilities, autism, physical or sensory impairments, mental health needs, or complex care requirements.

The home

Our most recent Care Quality Commission (CQC) inspection resulted in a ‘Good’ rating across all areas—a reflection of our high standards in safety, responsiveness, effectiveness, leadership, and care.
"People receive good quality care from kind and respectful staff who promote independence and dignity. The service is well-led with a strong commitment to continuous improvement.”
We’re proud of this recognition and continue to strive for excellence in every aspect of our work.
